Continuum of Care

The Milwaukee Continuum of Care is a coalition of local nonprofits, housing providers, government agencies, and supportive service organizations with the shared goal of ending and preventing homelessness throughout our community. Members of the Continuum of Care work collaboratively to create effective, data-driven strategies that help families and individuals move to stable housing, maximize self-sufficiency, and prevent a return to homelessness.

United Way of Greater Milwaukee & Waukesha County serves as the Milwaukee Continuum of Care’s Lead Agency and is responsible for leading and coordinating the community’s response to homelessness. They also serve as the Collaborative Applicant by applying for and overseeing federal grants on behalf of the coalition.

The Milwaukee County Housing Division operates as the Coordinated Entry Lead and is the main access point for people experiencing homelessness to connect with emergency shelters and housing resources. The Institute for Community Alliances manages Milwaukee’s Homeless Management Information System (HMIS), a secure database that tracks information about families and individuals who are experiencing or at risk of experiencing homelessness.

What is a Continuum of Care?

A Continuum of Care (CoC) is a regional or local planning body funded by HUD (U.S. Department of Housing and Urban Development) to coordinate housing and services for people experiencing homelessness. 

The CoC in Milwaukee is a collaborative made up of over 100 members that all have a common goal of working to end homelessness.

Key Aspects of a Continuum of Care 

  • Purpose: To promote community-wide commitment to ending homelessness by providing funding to nonprofit providers and local governments.
  • Target Population: Individuals and families experiencing homelessness, including those fleeing domestic violence, youth, and those with chronic homelessness conditions.
  • Goal: The primary goal is to provide a "continuum" of services—from shelter to permanent housing—to help people achieve long-term stability.
